Fall 2020 Golf Candidate Form

Any student who will be attending CHS in the fall of 2020 and is interested in trying out for the CHS Golf team should fill out the Google Form listed below.

Filling out this form does NOT register you for the program. An online registration will be made available in the coming months. For now, this form allows us to reach out to you with any important upcoming information.

Please fill out this form by June 15th. More information about the golf team, the upcoming season and the tryout process will be shared with you in the during the week of June 8th.
